- A rare, silver coloured metal medal with accompanying formal uniform ribbon.
- Together they comprise the award for outstanding blood donations in former Czechoslovakia.
- The ribbon is 3.5cm wide and has a smaller version of the medal on the ribbon face. This has a safety clasp fastener to the rear.
- The larger medal is 3.5cm wide by 7cm tall and has a safety clasp style fasten on the topmost section. The reverse side is blank.
- The medal text features Dr Jan Jansky who is remembered for his pioneering work on blood grouping and commemorates his life 1873 - 1921
